This financial estimate is intended to facilitate our ability to provide excellent service to you while minimizing our administrative costs.

All charges incurred are the patient's responsibility regardless of insurance coverage. We must emphasize our relationship is with you, our patient, not with your insurance company. Your insurance policy is an agreement between you, your employer, and the insurance company. If payment from your insurance company is not received within 45 days from date of service, you will be expected to pay the balance in full.

Your estimated copayment for treatment, which is the estimated amount not covered by your insurance, is due at the time treatment is provided.

Insurance will not cover LAB FEE COST. At Coastal Dental Arts we use the highest quality dental labs who produce and manufacture all crowns and related products here locally in San Diego, or in the CON US 48. We are able to provide a limited warranty on crowns made with our approved local lab. If you chose an alternative insurance covered lab, Coastal Dental Arts is not responsible for damages due to material failure once the crown is permanently delivered*.

*Patients on a regular recall schedule receive warranty on crowns for material defects, or breakage due to material defects for a minimum 3 years when a lab fee was applied.
